Getting Started
On your first visit to one of our Huntington Group offices, you will be asked to complete a new patient information and medical history form. You can either click on the Forms in our footer to download them before coming to our office or complete them once you get to the office, but please remember to bring a list of your current medications with you as we do ask for this information. This confidential information is most important in evaluation and treatment planning, so it is important that your answers be as comprehensive as possible. It may also be necessary to take new radiographs, impressions or photographs to complete your personal history. Minors must be accompanied by a parent or legal guardian, unless other arrangements have been made in advance.
Office Hours
Office hours vary with each office; however, one or more office is open every day Monday through Saturday. Our usual weekday hours are 8:30 a.m. to 4:30 p.m. and our usual Saturday hours are 7:30 a.m. to 11:30 a.m. Please call to check on specific hours to arrange an appointment. We do request you give us at least 24 hours notice if you are unable to keep your appointment.

Please arrive 30 minutes before your appointment time.
- Complete all office forms either at home or in the office.
- Bring all insurance cards with you (dental and medical)
- Bring a list of all current medications
- Bring any x-rays and referral slips from other doctors.
- Call our office promptly if you are unable to keep your scheduled appointment
Fees and Insurance Claims
Our policy is to provide the finest care at a reasonable cost. We want you to understand just what those costs will be. Our professional fees will, naturally, vary with the nature and complexity of your treatment. After our examination and treatment planning, we will be able to give a close estimate of your charges. In order to keep those charges as low as possible, we ask that payment be made at the time of service. This can be by cash, check, VISA, Discover or MasterCard. Ask our staff about Care Credit too.
We will be happy to file insurance claims for you. We feel that our fees are fair and reasonable; however, the amount your insurance company decides to pay may be less than our fee schedule. Our knowledgeable business staff will be able to answer questions that may arise. Please don’t hesitate to discuss financial arrangements with them at any time.
